This is the questionnaire of the Census of Manufacturing Establishments 1996/97 in Nepali version (not translated in English) language. In CME 1996/97, one single questionnaire was administrated for all types of establishments irrespective of their legal status, ownership and size of persons engaged and size of fixed assets with 16 sections.
At the right down corner of the cover page, a five digit unique identification number was designed by combining district code with serial number of enumerated establishments of corresponding district. Hence constructed unique identification number is defined as a variable "formno" which stands for "Form Number".
Four considerations to be given during interview are kept just after the cover page.
The First four sections of the questionnaire are introductory sections. The first section is designed to record the introductory information of the establishment including name, spatial address, registration number, phone number, post box number, fax number, e-mail address, date of operation and contact person name with address.Second and third section have one-one closed question having 6-6 options. They will give the legal status and ownership status of the establihsment respectively. Major industrial activity is kept in fourth section with NSIC code at four digit level.
Fifth section deals with employment and emolument. Fuel and industrial services are in section six and seven respectively. At the section 8 and 9, matrix of 9 x 16 and 8 x 16 each covering 2 pages are given to record the cost of raw materials purchased and value of shipments by country and CPC.
Section 10, 11, 12, and 13 possess stocks, non-industrial cost, indirect taxes and non-industrial receipts respectively having 6, 13, 5, and 7 sub-sections. The section 14 records fixed assets of establishments at the beginning of the year, addition of fixed assets during the year and sale, loss and depreciation of fixed assets during the year. Section 15 has a question for utilization of capacity. |
